In proceeding with services provided by Emma Barber, you are confirming that you give permission for the veterinary physiotherapist to discuss treatment with your registered veterinary surgeon and share personal information in accordance with the privacy statement and understand that the veterinary physiotherapist may refer my animal back to the veterinary surgeon for further investigation.

I hold information about you because you have booked a veterinary physiotherapy session for your animal. Personal data stored includes: Name, Address, phone number, email address.

This information is stored electronically or in hard copy and used to communicate with you by telephone and email. I will never contact you for marketing purposes. The legal basis for processing of your personal data, under the terms of the General Data Protection Regulation (GDPR), is that this processing is necessary for the purposes of the legitimate interests of Emma Barber as controller of the data collected. Your personal data will not be shared with a third party, other than for necessary contact with your veterinary surgeon, and will not be subject to any automated decision making or profiling.

Your rights: Your data will be stored for the purposes described above for a maximum of 10 years and you have the right to access the personal data I hold about you. You have the right to complain to your Statutory Authority (the Information Commissioner’s Office in the UK) about my handling of your personal data.

Failure to provide me with the data requested may result in me being unable to provide you with the services offered.
